In order for your child to attend CMS, you must first complete a registration packet and bring the following required forms to the office for confirmation. You can also purchase a school agenda, which each student is required to use daily to record homework and important dates. The front office staff will be happy to help you if you have questions. Required to Bring:
Proof of TDAP immunization - 7th only (if you have not already submitted to the school.)
PG&E bill to confirm residence (current and original)
