For new students entering Columbia Middle School, you must first bring in the following required information to the office.
You can also purchase a school agenda, which each student is required to use daily to record homework and important dates.
The front office staff will be happy to help you if you have questions.
Required to Bring:
PG&E bill to confirm residence (current and original)
One picture ID: Calif. Drivers License with current address or DMV stamped change of address.
Lease/rental agreement or mortgage statement
Car registration or payroll warrant or bank statement.
Original birth certificate or passport
Immunization record-Tdap proof for 7th grade. TB test if from out of Santa Clara Co.
